Services Provided: Personal Care Services and assistance with activities of daily living necessary to prevent elderly and disabled from entering nursing homes. Examples include cooking, grocery shopping, assistance with bathing and grooming, housekeeping, etc. Community Based Alternatives (CBA) program also offers home modifications and adaptive aids.

Requirements for Eligibility and Financial Guidelines: Eligibility determined by TDHS-CCAD caseworker including number of hours/week of service and specific tasks clients eligible for. Often eligible for Medicaid due to disability and income. Must be eligible for Medicaid to pay for their nursing home care if unable to stay at home.

Intake Procedure: Apply at local TDHS - CCAD office

Required Documents: Proof of income and assets or current Medicaid card.

Fee Structure: TDHS pays 100% of charges for eligible clients.
