Services Provided: Assistance with Activities of Daily Living similar to CCAD Program above. Skilled Nursing care. Occupational Therapy, Physical Therapy, Speech Therapy, Psychological Services, Respite, Adaptive Aids, and Home Modifications.

Requirements for Eligibility and Financial Guidelines: Must meet Medicaid Eligibility via SSI or traditional Medicaid Program. Must have incurred developmental delay prior to age 22 to be eligible. Typical clients have head injuries, spinal cord injuries, cerebral palsy, spina bifida, etc. Must have exhausted Voc Rehab, LISD Special Education, and Medicaid Comprehensive Care Program (CCP) prior to using CLASS program to pay for services these entities provide.
