Requirements for Eligibility and Financial Guidelines: TDH-PHC contract (55%) plus Fee for Service payments (45%). PHC Program assists with meds and lab for persons up to 150% of poverty. Medicare, Medicaid, and private payment are accepted for persons ineligible for the PHC Program.

Required Documents: Medicaid or Medicare Card. If requesting financial assistance bring proof of income.

Fee Structure: Private Physician office fee schedule. Financial assistance available to persons less than 150% of poverty.
