Requirements for Eligibility and Financial Guidelines: Homeless as defined in section 103 of
the McKinney Act. A homeless applicant is eligible for assistance under this program if his/her
income is at or below the current poverty income guidelines.
Intake Procedure: Applications are taken at each multi-service center. The center workers will
do home visits for shut-ins.
Required Documents: Proof of income, notice of mortgage foreclosure, eviction or termination
of utility services.

Other Comments: The contract is for 12 months, but the limited funding is usually spent in 2
or 3 months.
Individuals or families forced to vacate their fixed, regular residence due to extenuating
circumstances, and forced to accept shelter from a friend or relative, shall be defined as homeless
if the individual or family seeking services declared that they are homeless and that the shelter
being provided is temporary.
An applicant or family member who has received a legal or bonafide eviction notice and provides
an original thereof, containing applicant's name, address of residence from which the applicant is
being evicted, shall be defined as homeless and may be eligible for services on the date of
eviction given in the eviction notice.
Income eligibility: A homeless applicant is eligible for assistance under this program if his
income is at or below the poverty line for their family size as established annually by the
Department of Health and Human Services Poverty Income Guidelines.
